Submit one form per student:  For parents registering multiple children, please fill out and submit a separate form for each child.  Make note in the "Additional Comments Box" listing names of all children being registered so office may tie single payment to all registrants.  

​**On the first day of class, a legal guardian will be required to sign a Consent Form before your child may be able to participate.**

If you are registering your child for multiple programs at

different locations, specify the location & program pairings in the

**If you choose to enroll a single child in multiple programs, a $5 Dual Program credit will be applied to your child's class account for each additional program in which your child is enrolled.  This credit will be applied once registration payment is validated. **

 Registration fees must be paid to guarantee class

placement and for your child to

participate in class.
